The Practice has a current open list status and welcomes new registrations from individuals and families that have moved in to the Practice area. The Practice boundary is displayed on the notice board in the waiting area. If you are unsure whether your address is covered by the Practice, please give us a ring on 0151 949 2000 and a member of the Reception team will be happy to advise you.
All individuals and families wishing to register with the Practice will be required to complete individualised registration documents which includes a health questionnaire. Those over the age of 18 years will be required to provide two forms if identification. One form of photographic identity and one form of proof of address. A list of acceptable documents is available on the registration forms. For those aged under 18 years a copy of the birth certificate is required.
All newly registered patients aged 16 years and over will be offered a new patient health check appointment which is undertaken by our Healthcare Assistant. The aim of this appointment is to document some basic clinic information such as height, weight and blood pressure, all of which play a significant part in your clinical management.
If you are aged 75 years or over, you will be allocated a GP as per our in-house procedure. This GP will be responsible for overseeing your clinical care, but this does not stop you from seeing any GP in the Practice.

At registration, you will also be asked whether you wish to sign up for Internet Access.
